Waiver Form

Please make a point of reading this waiver in it's entirety before filling it out and signing it. A question that often comes up is who is required to fill out and sign a Waiver. Here is the simplest answer.

Anyone who is going to step aboard any Bimini Undersea vessel, for any reason must be accounted for on a waiver form.

It breaks down like this…

• All Adults (18 & older)– must fill out and sign their own individual waiver
• All minor children (17 & under) planning on SCUBA Diving
  • Must read, fill out, and sign their own waiver
  • Waiver must also be signed by parent or legal guardian
• Minor children travelling with a parent or legal guardian who are not engaged in SCUBA diving may be added to a parent/guardian's primary Waiver form. (Maximum 2 children added to any one parent's waiver)

Filling out the waiver is very simple. The only information you need to put on the form is your full name (as it appears on your drivers license or passport) and your Date Of Birth (aka DOB). The format for dates on the form is month/day/year (aka mm/dd/yy). If you were born on July 21st, 1973 you would put on the form 07/21/73. Once you have filled out all of the appropriate form fields, please print out the form, sign, and date it. Print out 2 copies if you wish to keep one for your records.

Submitting the forms

Once the waiver form has been filled in and printed, and signed, you have a couple of options as to how you may submit them to Bimini Undersea.

1. If you choose to submit them electronically, and, you have access to a scanner, simply scan the Waiver form(s) and email them as attachments to forms@biminiundersea.com . Be sure that you sign the waiver form before emailing it in.
2. The other option is to simply bring the completed and signed waiver forms with you and give them to the dive staff upon your arrival.

Either way, you still save considerable time over the traditional method of filling out forms upon your arrival.
